I’m grateful to technology and for being able to deal with all administration – counselling forms etc – online. It’s like having an assistant and there are no costs to pass on?!
Fill out the Intake Registration Form (below) and read and initial the Limits of Confidentiality (Form) and the Ethics Guidelines (Form) online and click ‘submit’. There is no need to print or scan anything: it can be done here and now, online. The forms are filed automatically and I receive a copy of the intake form. Thank you!

Your Intake automatically arrives in my inbox after you click ‘submit’ and you should receive a confirmation in your email.
Appointment and Payment
Online by Zoom: Please fill out the forms and send me an email suggesting 3 times that work well for you. I will reply and set up an appointment. You can pay online (by Credit Card or Paypal) or we can agree that I will send you an invoice after the session and you can send me an e-transfer.
Sidney or Langford: office counselling is available in Sidney and Langford and I will send you an invoice to pay online (by email e-transfer) after our session.
Check your spam and junk mail if you don’t see your emails or invoices: once we have sent emails back and forth a few times this won’t be an issue. Please’Whitelist’ me, ReBuildingYou, WordPress and Wave.com
Whitelisting ensures that email makes it to your inbox and doesn’t get held up by your junk or spam filters. If you don’t receive a reply – or copies of your forms – please check both your junk mail and your spam folders (most email accounts have both folders).
All 3 counselling forms should be filled out before our first session as these are the ‘admin’ we need to work together. Once these forms are filled out we can spend all our time working.
If we are meeting in my office, I would appreciate it if you would still complete the 3 Counselling Forms online as this saves counselling time. You are welcome to use the Introductory Email to set up our sessions – remind me which office you want to attend. I will send you an Invoice after our sessions.
If we are meeting online, please complete the 3 Counselling Forms and send me your Introductory Email when you like. Please Make payment or make your appointment for an Online Office session. Payment can be made online or let me know that you would prefer an email e-transfer invoice.
For email counselling, please complete the 3 Counselling Forms and send me your Introductory Email as soon as you like after making payment. I will always reply within 24 hours. Thank you!
- Click the cog icon in the top-right corner and then More mail settings.
- Select ‘Safe and blocked senders’ and then select ‘Safe senders’.
- Add add the domain of the email you want to whitelist to the list of Safe senders.
- To do this, add ReBuildingYou@gmail.com, WordPress and firstname.lastname@example.org (for invoices) as contacts in your address book. When an email comes in, your email provider will see that it’s a trusted contact and delivers the email to your inbox as opposed to your spam/junk folder.
If you still can’t find your copies of the counselling forms, which should arrive in your inbox, please send me an email and I can resend them.
