Counselling Forms: good for all counselling methods

I’m grateful to technology, being able to deal with administration – counselling forms etc – online.  It’s like having an assistant and there are no costs to pass on?!  Would you be willing, please, to fill out the Intake Registration Form (below) and to read and initial the Limits of Confidentiality (Form) and the Ethics Guidelines (Form) online and click ‘submit’. There is no need to print or scan anything: it can be done here and now, online. They are filed automatically and I receive a copy of the intake form.

  1. Intake Registration Form.
  2. Limits of Confidentiality Form.
  3. Code of Ethics Form.

No need to download, print and scan – everything can be done right here, online!

Your Intake automatically arrives in my inbox after you click ‘submit’ and you will receive a confirmation in your email.

Appointment and Payment

Please fill out the forms and send me an email suggesting 3 times that work well for you. I will reply and set up an appointment. You can pay online (by Credit Card or Paypal) or we can agree that I will send you an invoice after the session and you can send me an e-transfer.

For office counselling sessions I will send you an invoice to pay (by email) after our session and you can pay by e-transfer.

Please ‘Whitelist’ us, ReBuildingYou and WordPress, Wave.com so we don’t end up in your trash (and please check there for us if you don’t see my reply)!

Whitelisting

Whitelisting ensures that email makes it to your inbox and doesn’t get held up by your junk or spam filters. If  you don’t receive a reply – or copies of your forms – please check both your junk mail and your spam folders (most email accounts have both folders).

All 3 counselling forms should be filled out before our first session as these are the ‘admin’ we need to work together.  Most counsellors take admin time out of their sessions but by using technology we avoid all that. Once these forms are filled out we are ready to get started.

If we are meeting in my office, I would still appreciate it if you would complete the 3 Counselling Forms online. Either contact me (or I will contact you within 24 hours of receiving them) to set up an appointment. If you prefer, please feel welcome to send me an email and I will get back to you the same day. You are also welcome to use the Introductory Email to set up our sessions. I will send you an Invoice after our sessions.

If we are meeting online, please complete the 3 Counselling Forms and send me your Introductory Email as soon as you like after making payment and/or you make your appointment for an Online Office session. Payment can be made online or, if you prefer (please let me know that this is what you would like) I will send you an invoice.

For email counselling, please complete the 3 Counselling Forms and send me your Introductory Email as soon as you like after making payment. I will always reply within 24 hours. Thank you!

Please whitelist – or add – the address to your safe senders (Steps 1 and 2 are for Gmail…)
  1. Click the cog icon in the top-right corner and then More mail settings.
  2. Select ‘Safe and blocked senders’ and then select ‘Safe senders’.
  3. Add add the domain of the email you want to whitelist to the list of Safe senders.
  4. To do this, add ReBuildingYou@gmail.com ( and maildelivery@wave.com for invoices) as contacts in your address book. When an email comes in, your email provider will see that it’s a trusted contact and delivers the email to your inbox as opposed to your spam/junk folder.

If you still can’t find your copies of the counselling forms, which should arrive in your inbox, please send me an email and I can resend them/ we can check together.
