I’m grateful to technology to deal with all administration online – counselling forms etc.

Would you be willing please to fill out the Intake Registration Form (below) and read and initial the Limits of Confidentiality and the Ethics Guidelines (also below) online and click ‘submit’. There is no need to print or scan anything: it can be done right here. Online. The forms are filed automatically and I receive a copy of the intake form. Thank you!

  3. Code of Ethics Form.

No need to download, print and scan – everything can be done right here, online!

Your Intake automatically arrives in my inbox after you click ‘submit’ and the other forms are filed. You should receive copies of all the forms you complete in your email.

Appointment and Payment

Online Sessions by Zoom

Please fill out the forms and send me an email suggesting 3 times that would work well for you. I will reply and set up an appointment, guided by your suggestions.  I will send you an invoice after the session and you can send me an e-transfer. If you wish to purchase ConnectionCue or RelationshipCue cards before our session, please do so on this website. The cost is the same. I can also send them to you and charge for them on your invoice.

International sessions please use the website / Paypal / credit card form of payment and I will send you a receipt.

Office counselling is available in Oak Bay, Victoria and I will send you an invoice to pay online (by email e-transfer) after our session.

Check your spam and junk mail if you don’t see your emails or invoices: once we have sent emails back and forth a few times this won’t be an issue. Please’Whitelist’ me:, and


Whitelisting ensures that email makes it to your inbox and doesn’t get held up by your junk or spam filters. If  you don’t receive a reply – or copies of your forms – please check both your junk mail and your spam folders (most email accounts seem to have both folders).

Please whitelist – or add – the address to your safe senders list (Steps 1 and 2 are for Gmail…)
  1. Click the cog icon in the top-right corner and then More mail settings.
  2. Select ‘Safe and blocked senders’ and then select ‘Safe senders’.
  3. Add add the domain of the email you want to whitelist to the list of Safe senders.
  4. To do this, add, and (for invoices) as contacts in your address book. When an email comes in, your email provider will see that it’s a trusted contact and delivers the email to your inbox as opposed to your spam/junk folder.

If you still can’t find your copies of the counselling forms, which should arrive in your inbox, please send me an email and I will problemsolve and resend the copies.

All 3 counselling forms should be filled out before our first session as these are the ‘admin’ we need before we can work together.  If you have any questions arising from these forms, please feel free to email me before our session.

If we are meeting in my office, I would appreciate it if you would still complete the 3 Counselling Forms online as this saves counselling time and files them automatically. You are welcome to use the Introductory Email to set up our sessions. I will send you an Invoice after our sessions.

If we are meeting online, please complete the 3 Counselling Forms and send an Introductory Email so that we can make an appointment for an Online Office session. Payment can be made online or let me know if you would prefer an email e-transfer invoice.

For email counselling, please complete the 3 Counselling Forms and send me your Introductory Email after making payment. I will always reply within 24 hours. Thank you!
